The reporting
period for this report is January 1- December 31, 2020. All the work your
Chapter has done during this period will count towards this year’s reporting.
We are looking forward to seeing all the continued great work you are doing
within your Chapters. The applications are due to National on or
before June 15, 2021.
The membership
and certification numbers are embedded within the application in PDF format
(see screenshots below). They will open in any free or paid version of
Adobe. If you use an alternate PDF program, you’ll need to download the free version of Adobe Reader.
