12:05 - 1:05 p.m.     Outline best practices to improve outcome measures of bathing, bed transferring, ambulation, mgt of oral meds, and dyspnea. Explain process measure for timely initiation of care and best practices for intake, field staff and quality assurance staff to improve score.
1:05 - 1:25 p.m.       Outline best practices to reduce acute care hospitalizations.
1:25 - 1:30 p.m.       Q&A/Evaluation
