1. Identify one requirement for QAPI.
2. Name one data source to use for target selection.
3. Describe one way to evaluate if your QAPI plan is effective.

12:00 -12:10 p.m.  Review the standards under the Conditions of Participation for QAPI and Infection Prevention/Control. Explain Public Health Emergency impact on QAPI.
12:10 - 12:20 p.m.   Discuss data sources to be used for target selection, including CASPER reports, HH-CAHPS reports, benchmarking reports, past surveys, complaint log, chart auditing results, and incident reports.
12:20 - 1:25 p.m.     Review the requirements for QAPI action plan. Describe the processes for Root Cause Analysis and Rapid Cycle Improvement PDSA using a case example of an infection control target measure. Discuss how to evaluate the effectiveness of agency’s action plan, identify “pain points” where action plan fails.
1:25 - 1:30 p.m.       Q&A/Evaluation
